• View or request a copy of your laboratory results

• Ask us to send your results to another person you designate

• Request limits on how we use or disclose your PHI

• Request confidential communication (e.g., at a different address)

• Request an amendment if something in your record is incorrect

• Receive a list of certain disclosures we have made

To exercise any of these rights, please contact our Privacy Officer.

Breach Notification

If your unsecured PHI is ever compromised, SAMP Labs will notify you promptly as required by HIPAA.
